3. Top-of-car work — CWT top runby, top clearance, refuge space measurements	Fall from car top (558′ travel), struck by overhead structure (low clearance — only 40.75" and 62" to ceiling), pinch points from guide rails/rollers	1. Full-body harness — anchor to designated car top anchor point. 2. LOTO in effect — car on inspection mode only if movement required. 3. Inspector briefed on LOW OVERHEAD (40.75" clearance — crouch required). 4. No tools or materials on car top edge. 5. Hard hat Class E, safety glasses. 6. Stay within car top perimeter.
4. TLS slowdown limit switch inspection & adjustment	Electrical shock from TLS wiring (120V control voltage), car movement during inspection, falling while accessing limit switch tape along rail	1. LOTO on control circuit before touching TLS wiring. 2. Insulated tools rated for control voltage. 3. Rubber insulating gloves Class 00 minimum. 4. Work from stable platform — no reaching beyond safe reach envelope. 5. Second person stationed at machine room to prevent inadvertent energization.
5. Machine room — motor/controller inspection, DC switchgear	Arc flash from DC switchgear (high energy DC arc more dangerous than AC), electrical shock, rotating machinery (motor, sheaves)	1. Arc flash hazard analysis performed — use appropriate PPE per NFPA 70E (arc flash suit, face shield, insulating gloves). 2. LOTO on main disconnect before opening panels. 3. Machine room door closed when not in use. 4. Guards on rotating parts verified in place. 5. Minimum 2-person rule for electrical work.
6. Restore elevator to service — remove LOTO, test run	Personnel in hoistway/pit not notified, car movement with workers in hazardous position, incorrect restoration sequence	1. Verbal ALL CLEAR confirmed from all work locations before LOTO removal. 2. All workers must be at safe location (not in pit, not on car top) before energization. 3. HECP restoration sequence followed per FIST 1-1. 4. Slow speed test run with observer at bottom terminal. 5. Verify TLS limits respond correctly before returning to normal service.

Top of car work (Step 3) = HIGH risk due to extremely low overhead clearance (40.75"). Inspector must crouch — full fall arrest required. Consider whether overhead structure constitutes a code deficiency requiring written report.
